Photo #4 - Optional - Exterior Physical Damage/Defect - you may upload multiple images. This should only include conditions visible on the EXTERIOR of the product. DO NOT disassemble the product as this will void the warranty.

Photo #5 - Please upload an image of your store receipt or other proof of purchase. A screen shot of your order confirmation is acceptable. Be sure the purchase date is clearly visible in the photo.

Note: Our warranty covers the massager against manufacturer defect for one year from the date of purchase. No refunds, repairs or substitutions are covered under the warranty. Once your claim is approved we will ship a new replacement of the massager submitted.
